If the link says "page not found" while it is loading, wait up to 10 seconds. The form should load in.
All required information has an asterisk next to it and if you are missing even one required item, the form will not let you submit. After an unsuccessful submission, the form will let you know what is still missing at the top.
If your youth/client has been referred to The Covering House previously, the form will not allow you to submit. Please send an email so that we can contact you directly.
